Job Summary

We are seeking a highly skilled and compassionate Family Support Coordinator to join our team at Devereux Advanced Behavioral Health Arizona's Community Based Services program. As a key member of our team, you will play a vital role in providing in-home remedial intervention for at-risk youth and families, identifying strengths and needs to enhance the Child and Family Team (CFT) planning process.

Key Responsibilities
  • Coordinate care, treatment planning, service delivery, and case management of placement preservation and crisis stabilization services
  • Formulate treatment plans, assessments, clinical impressions, and discharge planning
  • Provide support to individuals, families, and placements to help stabilize behaviors and avoid disruption
  • Work closely with the Child/Family Team to identify supports needed to ensure stabilization and maintain stability
Requirements
  • Minimum requirement of high school diploma or equivalency
  • At least four (4) years of full-time behavioral health work experience
  • Valid Arizona Driver's License and pass DMV 36-month history check
  • Current AZ Fingerprint Clearance Card with no restrictions (or eligibility to obtain with no restrictions)
