About the Role

We are seeking a skilled Licensed Practical Nurse to join our team as a Hospice Nurse. As a member of our interdisciplinary team, you will provide direct patient care under the supervision of a registered nurse, following the plan of care and working collaboratively with the team to achieve positive patient outcomes.

Responsibilities
  • Provide direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
  • Implement current nursing practice following a comprehensive assessment and the plan of care.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
  • Participate in coordination of hospice services, appropriately reporting identified needs to the interdisciplinary group.
  • Use and prepare equipment and supplies effectively, adhering to aseptic technique.
  • Provide appropriate pain/symptom management, evaluating patient response to treatments/medications.
  • Participate in personal, professional growth and development, and organization's quality assessment performance improvement teams and activities.
Requirements
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years nursing experience, with community health/hospice or medical/surgical experience preferred.
  • Currently licensed as an LPN/LVN in California.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Possesses and maintains current CPR certification.
  • Understands philosophy of hospice concept and needs of the terminally ill.
