Hospice Nurse

The Hospice Nurse - Hybrid will play a vital role in providing high-quality care to patients and their families. This position requires a registered nurse with experience in nursing, community health education, and experience working with patients and families in their homes and communities.
Key Responsibilities- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide professional nursing care, including administering medications and treatments as prescribed by physicians.
- Counsel patients and families on meeting their nursing and related needs, and provide health care instructions as appropriate.
- Act as a case manager, coordinating patient care for assigned caseloads and working with the interdisciplinary team to meet the emotional needs of patients and families.
- Participate in on-call duties, ensure arrangements for equipment and other necessary items and services are available, and supervise ancillary personnel as needed.
- Graduate of a National League of Nursing accredited school of nursing.
- Current licensure in California and current CPR certification.
- Minimum of one year of experience as a professional nurse within the last three years, or a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license.
- Minimum of two years' experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.

This position operates on a hybrid schedule, working at home, in the field, and in-office. The office is located at 1680 S Garfield Avenue, Alhambra, CA 91801.
The total compensation target pay range for this role is: $50.00 per hour. The salary range represents our national target range for this role. We offer additional stipends for on-call/after-hours visits and mileage.
