**Job Summary**

We are seeking a skilled Patient Care Specialist to join our team at Precision Care Partners. As a Licensed Practical Nurse, you will play a vital role in delivering exceptional healthcare services and professional training that empower individuals and support our community.

Key Responsibilities:

  • Provide direct patient care, including administering medications, monitoring vital signs, and assisting with procedures.
  • Document patient information accurately and timely in electronic health record systems such as Athenahealth, eClinicalWorks, and Cerner.
  • Collaborate with healthcare team members to develop and implement patient care plans.
  • Educate patients and their families on health conditions, treatments, and preventive measures.
  • Assist with family planning services and provide education on contraception options.
  • Monitor and care for neonatal ventilator patients.
  • Provide care for toddlers, including administering age-appropriate vaccinations and conducting developmental assessments.
  • Ensure adherence to infection control protocols in sterile processing areas.
  • Perform basic IV therapy and suture removal.

Requirements:

  • Valid LPN license in the state of Virginia.
  • Knowledge of medical terminology, procedures, and standards of practice.
  • Familiarity with electronic health record systems such as Athenahealth, eClinicalWorks, and Cerner.
  • Ability to provide compassionate and patient-centered care.
  • Excellent communication and interpersonal skills.
  • Strong attention to detail and organizational abilities.

Salary: $65,000 - $75,000 per year.
