We are seeking a skilled LVN to join our team in providing high-quality care to patients in their homes or facilities. As a Continuous Care LVN, you will work under the supervision of an RN to deliver professional nursing care to patients across the lifespan.
Responsibilities- Assist RN in analyzing situations, anticipating problem areas, and taking appropriate action.
- Evaluate patient response to care, ensuring safe and timely outcomes.
- Perform licensed nurse skills, adhering to policies, procedures, and standards of care.
- Implement and document teaching plans, tailored to patient and family needs.
- Provide support to the agency in completion of assigned audits and office duties.
- Vocational Nurse Training
- Minimum 12 months of LVN experience, preferably in acute care, home health, or hospice
- Current California LVN license
- Familiarity with computerized bedside patient documentation
- Ability to function as part of a team and take direction from others
