We are seeking a skilled LVN to join our team as a Hospice LVN Continuous Care Specialist. As a member of our interdisciplinary team, you will provide professional nursing care to patients in their homes or facilities, working under the supervision of an RN.

Responsibilities:

  • Assist RN in analyzing situations and taking appropriate action.
  • Evaluate patient response to care and make adjustments as needed.
  • Perform licensed nurse skills and adhere to the plan of care for each patient.
  • Implement and document teaching plans based on patient and family needs.
  • Document all care provided to patients, including telephone interactions with other staff members.

Qualifications:

  • Vocational Nurse Training.
  • Minimum 12 months of experience as an LVN, preferably in acute care, home health, or hospice.
  • Current California LVN license.
  • Familiarity with computerized bedside patient documentation.
  • Ability to function as part of a team and take direction from other team members.
