Job Summary:

The Stamford Hospital is seeking a skilled Licensed Practical Nurse (LPN) to join our team at Harbor Point Family Medicine. As a LPN, you will play a vital role in coordinating patient care, providing mentorship to non-nursing personnel, and serving as a resource to staff and management.

Key Responsibilities:

  • Coordinate patient care, adhering to established standards of practice and state and federal guidelines.
  • Provide mentorship to non-nursing personnel, promoting a culture of excellence and teamwork.
  • Serve as a resource to staff and management, identifying and resolving problems in a timely and effective manner.
  • Check schedules and organize patient flow, greeting patients, documenting concerns, and preparing patients for physical exams.
  • Obtain and accurately document patient vital signs, weight, and report any abnormalities to physicians.
  • Assist with specimen collection and testing as necessary, documenting pertinent information.
  • Provide minor treatment as directed by physicians, assist with minor procedures, and provide education to patients and families as indicated.
  • Document all patient-related activities by the end of each shift.
  • Schedule patient testing and pre-certify if required by insurance.
  • Respond to all telephone messages by the end of each shift.
  • End-of-month checks for expired supplies and medication, stocking rooms, and maintaining inventory.
  • Clean exam and procedure rooms, equipment, and prepare for the next patient and physician needs.
  • Notify patients of test results, follow up, and attend scheduled staff meetings, in-service education opportunities, and continuing education programs.
  • Respect and protect patient confidentiality and privacy, disclosing information only for professional purposes.
  • Participate in performance improvement and risk management activities, completing required continuous training and education.

Requirements:

  • Graduate of an accredited school of nursing.
  • Valid license as a Licensed Practical Nurse in Connecticut.
  • Maintain a minimum of 6 contact hours annually.
  • Excellent customer service and communication skills.
  • Knowledge of medical terminology.
  • Minimum of 2 years of previous experience in a similar position.
  • Ability to read, write, and speak the English language.
