About the Role

We are seeking a skilled and compassionate Community Health Nurse to join our Adult Home Health team. As a member of our comprehensive team, you will deliver high-quality care to patients in their homes, providing a wide range of skilled nursing services while working to the top of your licensure.

Key Responsibilities

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
  • Collaborate with a multidisciplinary team to develop and implement patient care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.

Requirements

  • Greater than two years of home or public health nursing experience preferred.
  • Previous home care experience desired, but not required. We are willing to train and develop a candidate who demonstrates a willingness and desire to learn home health nursing.
  • Graduate nurses are considered for our Community Health Nurse I level.
