About the Role

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our team at Sangre de Cristo Community Care. As a Hospice CNA, you will play a vital role in providing comfort and support to our patients and their loved ones.

Responsibilities
  • Provide holistic care to hospice patients in their homes or residential facilities.
  • Assist patients with activities of daily living, including bathing, grooming, and toileting.
  • Monitor and report changes in patient's condition to the interdisciplinary team.
  • Offer emotional support and companionship to patients and their families.
  • Help maintain a clean, safe, and comfortable environment for patients.
  • Collaborate with nurses, physicians, social workers, and other team members to ensure coordinated care.

  • Competitive Salary - Minimum hourly wage of $17.00
  • Mileage reimbursement of sixty-two and a half cents per mile

Requirements
  • Valid certification as a Certified Nursing Assistant (CNA) in Colorado.
  • Valid non-restricted Driver's License
  • Compassionate demeanor and a genuine desire to make a difference.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively both independently and as part of a team.
  • Empathy, patience, and a non-judgmental attitude toward patients and families.
  • Prior experience in hospice care is preferred but not required.
