We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Chattanooga, Tennessee. As a LPN Homecare Specialist, you will provide high-quality patient care in the comfort of their own homes.

Job Summary

As a LPN Homecare Specialist, you will be responsible for delivering patient-centered care to pediatric and adult patients with various medical conditions. Your primary focus will be on providing skilled nursing services, including wound care, medication management, and vital sign monitoring.

Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Administer medications and treatments as prescribed by physicians
  • Monitor and document patient progress, including vital signs, medication administration, and treatment outcomes
  • Collaborate with healthcare teams to ensure seamless care coordination and communication
  • Provide education and support to patients and their families on disease management and self-care techniques
Requirements
  • Current LPN license in the state of Tennessee
  • One year of experience in home healthcare or a related field
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Strong problem-solving and critical thinking skills
