The ideal candidate will have experience in Discharge Planning in an Acute Care Hospital Setting and be able to drive patient progression to establish a discharge plan.

About the Role

The RN Case Manager will be responsible for coordinating care and collaborating with interdisciplinary teams to create, implement, and develop all aspects of discharge planning.

This includes identifying patients with post-acute care needs, anticipating and establishing a discharge plan, reviewing admission assessments, and removing barriers to care.

The successful candidate will be able to work effectively in a fast-paced environment, prioritize tasks, and maintain accurate records.

Requirements
  • An active RN license in DE or Compact State is required.
  • BSN required.
  • RNs with at least 2 years of medical-surgical nursing experience in an acute care hospital are encouraged to apply.
  • A minimum of 1 year of Care Management experience is strongly preferred.
