Job Summary:

Tri-FLEXSI Home Health Care, Inc. is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide high-quality, patient-centered care to our clients in their own homes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, following established care plans
  • Administer medications, dress wounds, and assist with other medical treatments as directed by the physician
  • Monitor and record patient's vital signs, progress, and any changes in condition
  • Collaborate with the physician and other healthcare team members to develop and update patient care plans
  • Educate patients and their families on medication management, disease prevention, and self-care techniques
  • Maintain accurate and complete patient records in accordance with state and federal regulations
  • Communicate effectively with patients, their families, and healthcare team members
  • Adhere to all company policies, procedures, and protocols
Requirements:
  • Current and valid Licensed Vocational Nurse (LVN) license in the state of Texas
  • Minimum of 1 year of experience in healthcare, preferably in a home health setting
  • CPR certification
  • Excellent clinical skills and knowledge of nursing practices and procedures
  • Empathetic, compassionate, and patient-centered approach to care
  • Ability to work independently and as a part of a team
  • Strong communication and interpersonal skills
  • Proficiency in electronic health records (EHR) systems
  • Must have a reliable mode of transportation and a valid driver's license
