Job Title: Early Childhood Development Specialist

Community Health Centers of Greater Dayton is seeking a dedicated Early Childhood Development Specialist to join our team. As a key member of our healthcare team, you will play a vital role in promoting the health and well-being of our patients, particularly in the area of early childhood development.

Key Responsibilities:
  • Assist patients in accessing care by utilizing resources such as transportation.
  • Work with patients to plan and monitor care, including reviewing charts prior to patient visits to identify open testing and referral orders.
  • Educate and assist patients and families in accessing relevant community resources.
  • Cultivate and support primary care and specialty provider co-management with timely communication, inquiry, follow-up, and reports.
Requirements:
  • Previous experience in a clinical or community resource setting.
  • Community Health Worker (CHW) Certification preferred, or Associates or Bachelors degree with previous Community Health Worker experience.
  • Knowledge of Community Health Centers preferred.

Physical Requirements:

Must be able to see, sit, stand, bend, stoop, hear, and lift up to 20 pounds. Must demonstrate manual dexterity in order to follow necessary procedures and documentation. Up to 15% travel.
