We are seeking a highly skilled and experienced RN MDS Coordinator to join our team at The Terraces of Boise, a HumanGood community. As an RN MDS Coordinator, you will play a critical role in ensuring the accuracy and completeness of resident assessments, as well as providing leadership and guidance to our interdisciplinary team.
Key Responsibilities- Resident Assessment and Care Planning: Develop, complete, and transmit resident assessments (MDS - Minimum Data Set) in accordance with current Federal and State standards.
- Utilization Review and Quality Improvement: Perform utilization reviews and have knowledge of Quality Improvement and Care Planning.
- Leadership and Guidance: Provide direct supervision to LVN MDS Nurses and lead PPS meetings to ensure skilled residents receive the appropriate care.
- Documentation and Compliance: Ensure PPS admission documentation compliance and forms are completed in a timely manner.
- Reporting and Analysis: Regularly review CMS reports, analyze data, and share with leadership team for quality improvement.
- Completion of Accredited Licensed Nursing Program (RN)
- Minimum three years' experience as an RN
- Minimum one-year experience as MDS Coordinator RN preferred
