About the Role

Key Responsibilities
  • Develop and implement individualized care plans in collaboration with physicians and healthcare professionals
  • Supervise and mentor home health aides and licensed practical vocational nurses as needed
  • Provide comprehensive assessments, including medication reconciliation, and accurately document patient visits
Requirements

To be successful in this role, you will need:

  • Current RN licensure in Arizona
  • Current CPR certification
  • A valid driver's license, vehicle insurance, and access to a reliable vehicle
