Job Summary

We are seeking a highly skilled RN Care Manager to join our team. As a critical member of our care team, you will work closely with complex renal patients in their homes, dialysis centers, and virtually to improve patient outcomes. Your primary focus will be to help patients achieve permanent access, promote home dialysis modalities, and educate patients on self-management.

Responsibilities
  • Conduct comprehensive assessments to identify gaps in care and barriers to good health
  • Create and implement care plans to address identified needs and improve patient health
  • Coordinate care across the continuum of care, including home, hospital, skilled nursing facility, and acute care facility
  • Manage patients during periods of transitions of care to minimize avoidable readmissions
  • Provide ongoing reassessment and follow-up to improve patient outcomes
Requirements
  • 1+ years of nursing experience in case management or care management
  • 2+ years of healthcare-related experience
  • Current, unrestricted compact Registered Nurse license
  • Strong analytical and critical thinking skills
  • Effective written and verbal communication skills
What We Offer
