Job Summary

The Member Health Navigator plays a critical role in establishing effective relationships with members to conduct health risk assessments and provide personalized support.

Key Responsibilities
  • Collaborate with the Integrated Care Coordination Team to ensure seamless care coordination and effective communication.
  • Coordinate logistics to support care plan goals and interventions, addressing immediate member concerns as needed.
  • Verify member eligibility and gather relevant information to inform care planning.
  • Conduct comprehensive assessments by gathering information from members, families, providers, and other stakeholders, as directed.
  • Contribute to care plan development and reporting, based on assessment outcomes and member needs.
  • Provide health education and coaching to empower members to take control of their health.
  • Maintain accurate documentation within established protocols and guidelines.
  • Empower members to feel informed, empowered, and supported by BCforward, by starting each interaction with a member-centered approach.
  • Identify opportunities for process improvement to enhance the member experience and streamline care coordination.
  • Perform other job duties as assigned to support the care management program.
Requirements
  • High School Diploma or equivalent required.
  • Minimum of two years of experience in community settings, working with at-risk populations, providing coordination of services, is preferred.
Competencies and Skills
  • Proficient in Microsoft Office, including Outlook, Word, and Excel.
  • Sensitivity to diverse cultures and experience working within different cultural contexts.
  • Excellent interpersonal skills and ability to work independently and within a team environment.
  • Ability to identify problems and opportunities and communicate effectively with management.
  • Developing knowledge of local, state, and federal healthcare laws and regulations and company policies regarding case management practices.
  • Demonstrate compassion, support, and collaboration with members and families.
  • Self-motivated and inquisitive with a willingness to learn and grow.
  • Ability to work in a fast-paced environment and maintain composure during high-stress situations.
  • Ability to maintain confidentiality and act in the best interest of the company.
  • Proven track record of demonstrating empathy and compassion for individuals and improving processes to enhance the member experience.
