We are seeking a dedicated and compassionate Registered Nurse to join our team as a Clinical Care Coordinator. In this role, you will be responsible for coordinating patient care, developing care plans, and collaborating with healthcare professionals to ensure that patients receive high-quality care.

Key Responsibilities
  • Coordinate patient care and develop individualized care plans
  • Collaborate with healthcare teams to ensure seamless delivery of services
  • Assess patients' needs and provide individualized support
  • Maintain accurate records of patient interactions and discussions with supervisors
Requirements
  • Currently licensed Registered Nurse (RN) in the state of employment
  • Previous experience as a Registered Nurse is preferred
  • Excellent communication and interpersonal skills
  • Valid driver's license
  • First aid/CPR certification required

Estimated annual salary: $75,000 - $95,000.
