**Job Description:**

We are seeking a skilled Speech Language Pathologist - Home Health Care Specialist to join our team in Abilene, TX. In this role, you will provide personalized therapy services to patients in their homes, enhancing their quality of life and promoting independence.

**Key Responsibilities:*

  • Evaluate and assess patient needs to determine the most effective care plan
  • Develop and implement patient-specific therapy plans, utilizing available resources and tools
  • Communicate effectively with patients, families, and healthcare teams to address care needs and ensure seamless delivery
  • Provide high-quality skilled care, supervising other care team members as needed
  • Document care delivery accurately and thoroughly in our electronic medical record (EMR) system

**Requirements:**

  • Masters degree in speech pathology from an accredited university
  • Current Speech-Language Pathology License, valid state driver's license, and auto liability insurance
  • 1 year of experience in acute care, rehabilitation, or home health settings preferred
