Job Title: Full Time LPN

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Actalent. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and support.

Job Summary:

The successful candidate will be responsible for conducting patient history intake and preparation, collecting and managing lab data, and conducting advanced telephone triage functions. You will also participate in research activities, act as a liaison between patients, families, and other healthcare professionals, and maintain accurate records and documentation.

Responsibilities:
  • Conduct patient history intake and preparation
  • Collect and manage lab data
  • Conduct advanced telephone triage functions and participate in research activities
  • Act as a liaison between patients, families, and other healthcare professionals
  • Maintain accurate records and documentation
  • Monitor and order supplies
  • Perform quality assurance (QA) functions as directed
  • Assist with the orientation and training of new personnel and clinical staff
Requirements:
  • State of MA LPN License
  • Associate's degree with LPN certification and 1-2 years related experience
  • One year of supervisory or human service experience is desired
  • Knowledge of and respect for diverse populations
  • Ability to travel within the region to access residential settings, employers, and community organizations
