Key Responsibilities:
- Provide high-quality patient care in accordance with established standards and protocols.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Document all patient interactions, assessments, and care plans in a timely and accurate manner.
Requirements:
- Graduate of an accredited nursing program with a current, unrestricted state license as a Registered Nurse.
- One year of experience working under current nursing license, with a focus on patient care and coordination.
- Current CPR certification and ability to travel to patient locations.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to lift 50 pounds and perform physical tasks as required by the job.
Preferred Qualifications:
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience and knowledge of relevant regulations and standards.
Physical Requirements:
- Ability to speak, write, read, and understand English.
- Ability to travel and perform physical tasks as required by the job.
