We are seeking a highly skilled and compassionate Registered Nurse to join our ACT Team at WellLife Network. As a key member of our team, you will provide high-quality nursing care to individuals with severe mental illnesses, working collaboratively with our multidisciplinary team to develop and implement individualized treatment plans.
Key Responsibilities- Conduct comprehensive assessments of clients' physical and mental health needs, developing and implementing individualized treatment plans in collaboration with the treatment team.
- Provide direct care to clients, including administering medications, conducting vital sign checks, and providing education on medication management and healthy lifestyle choices.
- Collaborate with the treatment team to develop and implement crisis intervention plans, providing support and guidance to clients and their families during times of crisis.
- Document all client interactions, progress notes, and treatment plans in a timely and accurate manner, maintaining confidentiality and adhering to agency and regulatory requirements.
- Participate in team meetings, providing input and feedback on client care and treatment plans, and contributing to the development of new policies and procedures.
- Develop and maintain relationships with community resources, including hospitals, pharmacies, and mental health providers, to ensure seamless coordination of care and services.
- Bachelor's degree in nursing and at least one year of experience working with individuals with severe mental illnesses.
- Clinical supervisory experience is desirable.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse populations and teams.
- Computer proficiency, including the ability to use electronic medical records and navigate software systems.
- Valid New York State driver's license and vehicle required.
