Job Summary

South Country Health Alliance is seeking a highly skilled Health Equity and Population Health Coordinator to join our team. As a key member of our Quality department, you will play a critical role in coordinating, communicating, and implementing health equity, social determinants of health, and population health management programs and projects.

This is an exciting opportunity to work with a dedicated team of professionals who are passionate about improving the health and well-being of Minnesota Health Care Program enrollees living in rural Minnesota communities.

Key Responsibilities
  • Coordinate and communicate with internal and external stakeholders to ensure successful implementation of health equity, social determinants of health, and population health management programs and projects.
  • Monitor compliance with state and federal regulations and assist with policies and procedures.
  • Develop and maintain partnerships with community groups to build health equity capacity and address health needs in our diverse membership.
  • Collect, organize, and analyze health care-related information, data, systems, and reports.
  • Conduct needs assessments and evaluations of quality programs and business partners to identify and address health equity, social determinants of health, and population health management initiatives.
Requirements
  • Bachelor's degree in health care administration, public health, business administration, social science, or related field.
  • At least 3 years of previous work experience in a managed care organization, public health, project management, quality improvement, or related work.
  • Experience with government programs, population health management, health equity, or social determinants of health.
  • Intermediate expertise with Microsoft Office Suite.
