Job Overview: Patient Flow Operations Coordinator

Full-time / Evening Shift

POSITION SUMMARY

The Patient Flow Operations Coordinator is tasked with overseeing the seamless movement of patients within the Emergency Department and Observation areas. This role also encompasses the management of patient transitions to outpatient surgical facilities.

This position is crucial in coordinating patient admissions from the Emergency Departments to other acute care facilities, ensuring efficient transfers and optimal patient care.

The Coordinator will work closely with physicians and administrative staff across all facilities to guarantee that patients receive the highest level of care until they can be safely moved to appropriate settings, including managing bed assignments and assisting with critically ill patients.

Additionally, the Coordinator will liaise with staff at other hospitals to ensure smooth transitions for patients requiring admission.


Key Responsibilities:
  • Oversee daily operations to facilitate efficient patient transfers to various care settings, including the Observation Unit and acute care facilities.
  • Act as a liaison with physicians regarding patient disposition plans and ensure timely updates to care plans.
  • Lead nursing staff to prioritize patient flow effectively.
  • Monitor room availability and turnover rates to maintain optimal patient flow.
  • Notify clinical staff of upcoming patient dispositions.
  • Facilitate patient movement to designated areas efficiently.
  • Manage tracking systems to keep patient data current and accessible.
  • Establish priorities for patient admissions and discharges.
  • Collaborate with administrative and management teams to address barriers affecting patient flow.
  • Participate in daily huddles to discuss patient census, length of stay, and staffing needs.
  • Provide clinical guidance and support to all caregivers as needed.
  • Work with Charge Nurses to identify patients suitable for the Observation Unit.
  • Report on patient flow processes and team performance in relevant meetings.
  • Address throughput challenges promptly and implement process improvements.
  • Ensure adherence to quality standards regarding patient wait times.
  • Manage patient relations issues to enhance satisfaction and safety.
  • Consult with admitting physicians to evaluate inpatient status for emergency patients.
  • Coordinate with various departments for effective patient placement.
  • Ensure thorough documentation for critical patient conditions prior to transfer.
  • Conduct safe and effective handoffs of patient admissions to other clinical staff as necessary.
  • Maintain availability for communication throughout the shift.
  • Perform additional related duties as assigned.

Experience:

Required:
5 years of clinical experience, including 3 years in an Emergency Department and 2 years in Critical Care, along with 2 years of management experience.

Preferred:
3 years of nursing leadership experience in Emergency and Critical Care Services.

Skills:

  • Proficient in Microsoft Office Suite.
  • Basic knowledge of medical coding and terminology.
