LPN Supervisor

We are seeking a skilled LPN Supervisor to join our team at Ontario Center for Rehabilitation and Healthcare.

• Conduct regular facility rounds to ensure a safe and healthy environment for residents.
• Monitor the assignments of aides to ensure proper workflow and efficiency.
• Supervise medication assistance times to ensure accurate recording and follow-up on any issues.
• Intervene in resident conflicts and behavior issues, and facilitate communication to address medical and mental health situations.
• Ensure all incident reports are completed and summarized for the EHP Coordinator/Administrator.
• Attend to late admissions and re-admissions, including paperwork and medication changes.
• Communicate shift activity through 24-hour reports to be reviewed by the Enriched Housing Program Coordinator/Administrator.

• Ability to work effectively as a team member.
• Valid state LPN license.
• Good standing with the State Registry.
