LPN Supervisor

We are seeking a skilled LPN Supervisor to join our team at Ontario Center for Rehabilitation and Healthcare. As a key member of our healthcare team, you will be responsible for providing exceptional care and support to our residents.

• Conduct regular facility rounds to ensure the well-being of our residents.
• Monitor the assignments of aides to ensure proper workflow and efficient care delivery.
• Supervise medication assistance times to ensure accurate recording and follow-up as needed.
• Intervene in resident conflicts and behavior issues in a timely and professional manner.
• Facilitate communication to address medical and mental health situations promptly.
• Ensure all aspects of facility incidents and accidents are completed and reported to the EHP Coordinator/Administrator.
• Attend to late admissions and re-admissions, including paperwork and medication changes.
• Communicate shift activity via 24-hour reports to be reviewed by the Enriched Housing Program Coordinator/Administrator.

• Must be able to work effectively as a team member.
• Valid state LPN license and good standing with the State Registry.
