Role Overview: As a Rehabilitation Specialist, you will provide and oversee therapeutic services for residents, focusing on assessment, treatment, program development, and implementation, along with necessary documentation and communication.

Key Responsibilities:

  • Assess residents to identify those requiring skilled therapeutic intervention.
  • Document daily care in accordance with established protocols.
  • Deliver therapeutic services and initiate referrals when necessary, adhering to departmental guidelines.
  • Conduct regular screenings to evaluate residents' needs for treatment.
  • Gather data through evaluations to inform treatment planning and execution.
  • Monitor residents' responses to interventions and adjust treatment plans as needed to achieve specified goals in line with physician directives.
  • Design suitable home or community programs to support residents in enhancing their functional abilities.
  • Review the quality and relevance of services provided, ensuring effectiveness and efficiency based on predetermined standards.
  • Develop, assess, plan, implement, and follow up on individualized therapy programs.
  • Tailor treatment approaches to meet the unique needs of each resident.
  • Engage in and contribute to the Continuous Quality Improvement Program as directed.
