Key Responsibilities
  • Conduct In-Home Health Assessments: Evaluate patients' vital signs, medication adherence, and overall health status to identify areas for improvement.
  • Develop Personalized Care Plans: Collaborate with the healthcare team to create individualized care plans that meet patients' unique needs and goals.
  • Administer Medications and Treatments: Safely administer medications, treatments, wound care, and medical procedures following prescribed protocols and care plans.
  • Monitor Patient Condition: Regularly monitor and document patient condition changes, promptly communicating updates to the healthcare team.
  • Education and Support: Educate patients and their families on disease management, medication adherence, and lifestyle adjustments to promote wellness.
  • Collaboration and Communication: Work collaboratively with various healthcare professionals, including physical therapists, occupational therapists, and social workers, to ensure a holistic approach to patient care.
Requirements
  • Current LVN License: Possess a current LVN license in the state of Texas.
  • Home Health Experience: Proven experience as an LVN, preferably in a home health setting.
  • Clinical Assessment and Patient Care: Demonstrate proficiency in clinical assessment and patient care.
  • Strong Communication and Interpersonal Skills: Possess strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
  • Ability to Work Independently: Ability to work independently and as part of a team to achieve patient care goals.
  • Valid Driver's License: Possess a valid driver's license and reliable transportation.
