Job Summary

As a Licensed Vocational Nurse (LVN) with Embrace Hospice & Home Health, you will play a vital role in providing high-quality patient care in the comfort of their own homes. Our team of dedicated professionals is committed to delivering exceptional service and support to our patients and their families.

Responsibilities

Provide direct care to patients in accordance with a comprehensive care plan in the home care setting
Document patient services accurately and in a timely manner
Assess and educate patients and their families/caregivers on various aspects of care
Participate in the coordination of home care services and quality assessment activities
Ensure compliance with all state and federal legal and regulatory requirements

Qualifications

Graduate of an accredited practical or vocational nursing program with current licensure to practice in the state of operation
1 year of nursing experience required, 3+ years preferred
Experience in community health, hospice, or med/surg preferred
Commitment to clinical and documentation excellence
