Your Role:

As an MDS Coordinator, you will play a crucial role in ensuring the accurate and timely execution of the RAI process and care management from admission through discharge. This includes adherence to company policies and compliance with federal, state, and certification guidelines.

Key Responsibilities:

  • Collaborate with the Interdisciplinary Team to evaluate resident needs and schedule MDS assessments and care plan reviews as mandated by regulatory agencies.
  • Guide the Interdisciplinary team in determining the necessity of additional MDS assessments based on clinical criteria.
  • Facilitate daily meetings to review resident care and establish assessment reference dates.
  • Ensure compliance with all federal and state regulations concerning the RAI process.
  • Monitor MDS and care plan documentation for accuracy and completeness in the medical records.
  • Maintain up-to-date MDS status for assigned residents in accordance with applicable guidelines.
  • Conduct accurate data entry of resident information into the designated MDS software.
  • Perform thorough coding of the MDS using information from medical records, observations, and interviews.
  • Participate in interdisciplinary team meetings and quality assurance activities to share information and updates.
  • Assist in identifying significant changes in resident status and ensure MDS and care plans reflect these changes.
  • Organize scheduling and communication for resident care planning conferences.
  • Stay informed about advancements in data entry and computer technology.
  • Submit required documentation electronically to state databases as per company policy.
  • Ensure timely submission of MDS to the state and maintain organized validation records.
  • Engage in the regulatory survey process and address any identified deficiencies.
  • Report audit trends to the Quality Assurance Committee.
  • Oversee the completion and timeliness of the RAI Process from MDS to care plan completion.
  • Initiate and track RAI process documentation through the Point Click Care system.

Qualifications:

  • Graduate of an accredited Licensed Vocational program and licensed in the state of practice.
  • Preferred: Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
  • Strong understanding of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
  • Comprehensive knowledge of the Quality Indicator process and OBRA regulations.
  • Familiarity with the care planning process.
  • Experience with MDS 3.0 is preferred.
