Job Summary

We are seeking a skilled Social Worker to join our team at Telecare Corporation. As a Social Worker, you will play a vital role in providing general casework services to our members served and their natural supports.

Key Responsibilities
  • Develop and implement person-centered recovery plans in collaboration with members served and multidisciplinary treatment teams.
  • Provide education and support to members served and their families regarding mental health conditions and treatment options.
  • Assist with discharge planning, including assessing readiness for discharge, making referrals for placement and aftercare services, and coordinating with the Business Office to ensure benefits and finances are in order.
  • Develop and maintain an awareness of community resources to support members served.
  • Ensure members served understand their legal status and rights, and track legal status on all assigned members served.
Requirements
  • Bachelor's Degree in Social Work or Psychology from an accredited college or university.
  • Demonstrated experience with behavioral health services.
  • CPR, Crisis Prevention Institute (CPI), and First Aid certification.
  • Ability to work effectively as a member of a multidisciplinary team.
