Job Summary

Telecare Corporation is seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing clinical and casework services to our members served and their natural supports.

Key Responsibilities
  • Provide person-centered recovery planning and collaborate with other services and agencies to ensure comprehensive care.
  • Work under the supervision of a licensed clinician to establish and maintain therapeutic relationships with members served.
  • Develop and implement individualized treatment plans, providing individual and group therapy as needed.
  • Collaborate with multidisciplinary treatment teams to ensure seamless care coordination and linkages to community services.
  • Document all treatment provided in a thorough and consistent manner, ensuring accurate and timely reporting.
Requirements
  • A Master's Degree in Social Work from an accredited graduate school and working towards licensure.
  • One year of direct service experience with individuals with severe mental illness in an inpatient or outpatient setting.
  • Criminal justice clearance and education verification.
  • Must be at least 18 years of age and possess a valid driver's license.
Preferred Qualifications
  • Bilingual Spanish speaking skills.
  • Previous experience working with the population to be served and/or in a similar program setting.
Skills and Abilities
  • Ability to complete social histories, risk assessments, mental health status exams, and plans of care.
  • Ability to work collaboratively with physicians, nurses, and rehabilitation therapists.
  • Ability to form a therapeutic alliance with members served and families.
  • Sufficient medical knowledge to assess the needs of members served.
Work Environment

The physical demands of this position require the ability to sit, stand, walk, bend, push, pull, squat, kneel, and lift and carry items weighing 25 pounds or less. The position also requires manual dexterity, repetition, and the ability to work in a fast-paced environment.
