Key Responsibilities- Collaborate with Physicians and Healthcare Professionals: Work closely with physicians, staff, and other healthcare professionals to ensure seamless patient care and a smooth healthcare continuum.
- Coordinate Patient Wellness and Chronic Illness Management: Assist with coordinating patient wellness and chronic illness management for the clinic, ensuring that patients receive the best possible care.
- Supervise Clinical Care Delivery: Work with the clinic manager to supervise the delivery of clinical care, ensuring that high-quality standards are maintained.
- Communicate Effectively: Demonstrate effective communication skills, using written and verbal skills to communicate with patients, staff, and other healthcare professionals.
- Teach and Educate: Demonstrate effective teaching techniques, applying adult learning principles to educate patients and their support systems.
- Education: Graduate of an accredited program in Licensed Practical Nursing.
- Certification/Licenses: Licensed by the State of New Mexico as an LPN. BLS Certification issued through the American Heart Association required, or within 2 weeks of hire date.
- Skills: Knowledge of good business English, spelling, arithmetic, and practices. Ability to communicate effectively and demonstrate effective teaching techniques.
- Experience: One year of experience working as a nurse in a clinical setting.
- Environment: Work in a clean, well-lighted, smoke-free environment.
- Physical Requirements: Requires full range of body motion, including handling and lifting patients, manual and finger dexterity, and eye-hand coordination. Requires standing and walking for extensive periods of time.
