RN/LPN Long Term Care Specialist

The Good Samaritan Society is seeking a skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team in a Long Term Care setting. As a member of our nursing team, you will be responsible for providing high-quality patient care and collaborating with other healthcare professionals to ensure the best possible outcomes for our residents.
Key Responsibilities- Assess, plan, and implement individualized care plans for residents
- Collaborate with interdisciplinary teams to coordinate care and services
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Document resident progress and communicate with families and healthcare teams
- Participate in quality improvement initiatives and contribute to a culture of excellence
- Graduate of an accredited nursing program (RN or LPN)
- Current licensure as a RN or LPN in the state of practice
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Strong communication and interpersonal skills
