We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Healthcare Professional, conducting home risk assessments in various locations across Hackensack, Minnesota.
This is a PRN/coverage opportunity with flexible scheduling, offering competitive pay and mileage reimbursement for completed assessments. As a member of our team, you will have the opportunity to make a meaningful impact on the lives of others while working collaboratively with healthcare professionals and patients.
About the Position- Clinical responsibilities include conducting comprehensive health assessments, medical history reviews, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix Medical Network guidelines and protocols.
- You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Conduct adult/geriatric assessments, medical history reviews, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as necessary.
- Prioritize patient care and provide excellent customer service, ensuring timely completion of assessments and high-quality outcomes.
- Maintain accurate records and documentation, adhering to Matrix policies and procedures.
- Master's degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP/PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
- A minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant, with strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills, with a positive and outgoing personality and the ability to work effectively as part of a team.
- $110.00 per completed assessment (25 hours or more/month), with flexible scheduling and mileage reimbursement for all miles traveled when 20 or more assessments are completed monthly.
- No show payment for 10 or more booked assessments/month.
- Flexibility to work varied shifts, including days, evenings, nights, and weekends.
