Job Title: Medical Social Worker

Join CenterWell Home Health as a Medical Social Worker and be part of a team dedicated to providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.

About the Role:

We are seeking a skilled Medical Social Worker to join our interdisciplinary care team. As a Medical Social Worker, you will work closely with patients, families, and healthcare professionals to assess and address the social and emotional needs of our patients.

Key Responsibilities:
  • Evaluate and develop personalized plans of care to meet the emotional and social needs of patients
  • Assess and address any relationships between medical and nursing needs, financial resources, and available community resources
  • Provide education and support to patients and families to help them cope with social and emotional responses to illness or injury
  • Collaborate with the interdisciplinary care team to develop and implement plans of care
  • Supervise and evaluate the performance of Social Work Assistants (BSW) to ensure compliance with company, government, and professional standards
Requirements:
  • Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education
  • Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice
  • Knowledge of and ability to assist with discharge planning needs and obtain community resources
  • Ability to be flexible in work hours and travel locally
  • Must read, write, and speak fluent English
