Job Summary

We are seeking a skilled Medical Social Worker to join our interdisciplinary care team at CenterWell Home Health. As a Medical Social Worker, you will play a vital role in evaluating and developing personalized plans of care for our patients, addressing their emotional and social needs, and connecting them with community resources.

Key Responsibilities
  • Evaluate patients' social and emotional states, identifying areas of need and developing plans to address them.
  • Assess patients' relationships with their medical and nursing needs, financial resources, and available community resources.
  • Provide education and support to patients and families, empowering them to manage their care and make informed decisions.
  • Collaborate with healthcare teams to develop and implement comprehensive care plans, ensuring seamless transitions and optimal patient outcomes.
  • Supervise and mentor Social Work Assistants, ensuring high-quality care and adherence to company standards.
  • Maintain accurate and detailed documentation, including case conferences, patient/physician community contacts, visit reports, progress notes, and other relevant information.
Requirements
  • Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education.
  • Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
  • Knowledge of and ability to assist with discharge planning needs, and to obtain community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility).
  • Ability to be flexible in work hours and travel locally.
  • Must read, write, and speak fluent English.
