Job Summary: We are seeking a highly skilled Speech Language Pathologist to join our team at Patient Care Medical Group. As a Geriatric Care Specialist, you will be responsible for providing outpatient rehabilitation services in an in-home setting for the older adult population.

Key Responsibilities:

* Evaluate and treat patients in their homes, assessing their need for speech therapy interventions
* Develop and implement individualized therapy programs to restore, reinforce, and enhance performance and optimize independence
* Instruct, educate, and train patients and caregivers in skills and functions essential for promoting quality, independence, and productivity
* Make clinically necessary referrals to other disciplines within the care network as well as other agencies to address the most complex and comprehensive needs of patients

Requirements:

* Valid State Speech Language Pathologist and Audiology license/certification
* Cardio-Pulmonary Resuscitation (CPR) certification
* At least 1 year of geriatric experience
