We are currently seeking a skilled and compassionate Hospice LVN to join our growing team.
Responsibilities- Provide direct skilled bedside nursing care to hospice patients in their homes, adhering to the plan of care and physician's orders.
- Collaborate with the interdisciplinary team to assess patient needs, develop care plans, and monitor treatment effectiveness.
- Administer medications and treatments as prescribed by the physician, ensuring accurate documentation and patient safety.
- Educate patients and families on hospice care principles, symptom management, and available resources.
- Maintain clear and concise communication with the healthcare team, documenting all patient interactions and observations.
- Current license as a Vocational Nurse in the State of California.
- Proven experience providing hospice care or similar clinical experience.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team in a compassionate and supportive manner.
- Valid driver's license, reliable transportation, and willingness to travel to patient homes.
