Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work with a diverse range of patients and provide comprehensive health assessments, medical history reviews, and treatment plans. You will also have the chance to collaborate with our team of healthcare professionals to deliver coordinated care and improve patient outcomes.

Responsibilities
  • Conduct comprehensive health assessments and medical history reviews for patients in their homes, skilled nursing facilities, and other community settings.
  • Develop and implement treatment plans in collaboration with our team of healthcare professionals.
  • Provide education and support to patients and their families to promote healthy behaviors and improve health outcomes.
  • Collaborate with our team to deliver coordinated care and improve patient outcomes.
  • Document patient information and treatment plans in accordance with Matrix Medical Network's policies and procedures.
Requirements
  • Master's degree in Nursing or related field.
  • Current RN and NP licensure in state of practice.
  • Board certification in a Matrix-approved specialty.
  • Minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with electronic health records.
  • Excellent verbal and written communication skills.
