Individual Support Facilitator

Position Overview
Community Options, Inc. is a prominent national non-profit organization dedicated to providing essential services to individuals with disabilities across multiple states.We are currently in search of Full-Time and Part-Time Day Support Specialists. This position is designed to deliver personalized assistance aimed at helping participants navigate and overcome the challenges faced by individuals with disabilities in a community setting.
Compensation: Starting pay is set at $19.00 per hour.
Incentives: A sign-on bonus of $1,000 is available for full-time staff and $500 for part-time staff (16+ hours per week), eligible after 90 days of satisfactory employment. Additionally, a referral bonus of $700 for full-time employees and $350 for part-time employees is offered, contingent upon meeting the 90-day satisfactory work and attendance criteria.
Key Responsibilities:
- Empower and motivate individuals to enhance their independence.
- Utilize personal transportation for work-related duties.
- Engage in one-on-one interactions with clients.
- Conduct regular meetings with participants in various settings, including their homes and community locations.
- Work collaboratively with Case Managers to design and implement programs that address the social, emotional, physical, and personal needs of individuals.
- Provide continuous support for community integration initiatives.
- Track and document the progress of individuals in achieving community integration.
- Assist in fostering a supportive network between individuals and their communities.
- Ensure completion of medical consultation forms by physicians for all medical appointments.
- Administer medications as required and maintain accurate documentation.
- Organize leisure and volunteer opportunities for participants.
Qualifications:
- High School Diploma or GED is required.
- A valid Driver's License is necessary.
