About the Role
As a Hospice Nurse with Enhabit Home Health & Hospice, you will work with patients, their families, and other healthcare professionals during the final stages of patients' lives. You will provide compassionate care in private homes, residential care facilities, nursing centers, and other hospice care environments. You may also supervise licensed vocational/practical nurses and nursing aides.
Responsibilities
- Provide high-quality, patient-centered care in a variety of settings
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
- Conduct thorough assessments and evaluations to identify patients' needs and preferences
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
- Supervise and mentor licensed vocational/practical nurses and nursing aides as needed
Requirements
- Graduate of an approved school of nursing and licensed as a Registered Nurse in the state of employment
- Valid state driver's license, automobile liability insurance, and dependable transportation
- Minimum of one year of clinical experience (strongly preferred)
- Previous home health or hospice experience (preferred)
