George Junior Republic is seeking a skilled Clinical Case Manager to provide high-quality services to families involved with the Department of Child Services and/or Juvenile Probation. As a key member of our team, you will be responsible for delivering home-based and community-based services that promote positive outcomes for at-risk youth.
Key Responsibilities- Maintain confidentiality and comply with HIPAA regulations to ensure the highest level of client trust and respect.
- Deliver services in accordance with Indiana Department of Child Services Service Standards, utilizing strength-based approaches to enhance protective factors for clients.
- Collaborate with clients, families, and community partners to develop and implement individualized service plans that promote resilience, social and emotional competence, and child development knowledge.
- Conduct regular home visits and community-based assessments to identify client needs and develop strategies for improvement.
- Provide education and support to clients on parenting skills, child development, and social and emotional competence.
- Bachelor's or Master's degree from an accredited university in a related field (e.g., social work, psychology, education).
- Minimum of a High School Degree, GED, or Associate Degree required.
- Ability to work a non-traditional schedule, including evenings and weekends, as needed.
- Frequent travel within the service area, with occasional overnight stays.
