We are seeking a skilled Home Care Registered Nurse to join our team in the Hartford region. As a Home Care Nurse Specialist, you will provide primary nursing care to patients in their homes, working closely with physicians and other healthcare professionals to develop and implement personalized care plans.

Key Responsibilities
  • Assess patient conditions and develop individualized care plans in collaboration with physicians and case managers.
  • Provide high-quality nursing care to patients in their homes, including wound care, medication management, and education on self-care.
  • Collaborate with home health aides and licensed practical nurses to ensure seamless delivery of care.
  • Communicate effectively with patients, families, and healthcare teams to promote optimal health outcomes.
Requirements
  • Bachelor's degree in Nursing (BSN) or equivalent experience.
  • Licensure as a Registered Nurse in the State of Connecticut.
  • At least one year of experience as a professional acute care nurse.
  • Home care experience preferred but not required.

  • A competitive salary range of $75,000 - $90,000 per year, depending on experience.
