Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager. In this role, you will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and ensuring seamless coordination of services.
Key Responsibilities:- Provide direct patient care services, including assessments, care planning, and implementation of individualized plans of care.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure comprehensive care.
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs, and develop plans to address these needs.
- Coordinate with community resources, including home health agencies, hospitals, and other healthcare providers, to ensure continuity of care.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and services.
- Current RN licensure in the state of employment.
- 1+ year of experience in hospice, home health, or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to patient care.
