We are seeking a compassionate and skilled Clinical Care Coordinator to join our team at BayMark Health Services. As a Clinical Care Coordinator, you will play a vital role in ensuring the maintenance of quality care and monitoring the treatment of all patients.
Key Responsibilities- Ensure the maintenance of quality care and monitor treatment of all patients.
- Conduct internal program review, patient records review, and educational counseling related to substance abuse disorders.
- Assess training needs of clinical and support staff and report to the Program Director.
- Ensure staff compliance with Federal and State regulations governing the treatment of patients.
- Attend conferences, meetings, and training programs as needed.
- Provide intake/readmission services, follow-up calls, and crisis intervention/supportive services to patients as needed.
- Collect patient program fees and maintain a chart monitoring system.
- Develop professional relationships with patients and function as a team leader.
- Participate in and/or help perform and attend monthly Clinic In-service Trainings.
- LADC with Masters degree in behavioral health, and/or LCMHC required.
- Appropriate work experience in the behavioral health field, with at least five (5) years experience in substance abuse and one (1) year of experience in administering a substance abuse treatment agency.
- Satisfactory drug screen and criminal background check.
