RN Care Coordinator

BAART is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Care Coordinator. As a key member of our Vermont Hub and Spoke care team, you will play a vital role in providing specialized nursing and care management to support patients in recovery from opioid addiction.
Job Summary:We are looking for a dedicated and organized RN to coordinate care for patients receiving Medication Assisted Therapy (MAT). The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a team environment.
Responsibilities:- Coordinate care for patients receiving MAT, including health home services and comprehensive care management.
- Work collaboratively with a Clinical Care Coordinator to provide health home services to a panel of 100 MAT patients.
- Develop and implement individual plans of care, ensuring active patient involvement and coordination with services and supports across treatment and human services settings.
- Provide health promotion activities that empower patients to make informed decisions about their treatment and health.
- Coordinate comprehensive transitional care, ensuring seamless transitions between treatment settings and levels of care.
- Support individuals and families in their recovery journey, reducing barriers to access and promoting community participation and well-being.
- Track and report Health Home Quality Measures in collaboration with the care team.
- Maintain confidentiality, professional demeanor, and appearance at all times.
- Graduate of an accredited school of nursing with a valid Vermont State Registered Nurse License.
- Five years of clinical nursing experience, with a preference for experience in mental health settings or substance treatment programs.
- Ability to establish a therapeutic relationship with patients and support resources.
- Strong organizational skills and ability to manage numerous cases effectively.
- Skilled at process and clinical outcomes improvement using quality improvement methodologies.
- Ability to effectively use Microsoft Office suite of products and multiple electronic medical records systems.
