Job Summary

We are seeking a skilled Occupational Therapist to join our Home Health team at Providence Humboldt Home Health. As an Occupational Therapist, you will play a vital role in helping patients maintain or improve their level of independence.

Key Responsibilities
  • Develop, coordinate, and maintain therapeutic services and activities to support patients' rehabilitation and recovery.
  • Collaborate with an interdisciplinary team to provide comprehensive care and achieve optimal patient outcomes.
  • Conduct thorough assessments and create personalized treatment plans to address patients' unique needs and goals.
  • Provide education and training to patients and their families on proper techniques and strategies for maintaining independence and managing daily activities.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Current licensure as an Occupational Therapist in the state of California.
  • Ability to work independently and as part of a team to achieve patient-centered care.
  • Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare professionals.
  • Strong problem-solving and critical thinking skills to address complex patient needs and develop creative solutions.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.
