Hospice Care Position Available

Altitude Hospice seeks a dedicated Licensed Practical Nurse (LPN/LVN) to join our team. In this role, you will have the opportunity to work closely with patients and their families, providing emotional support and physical care.

Essential Duties:

  • Develop and implement individualized care plans that cater to each patient's unique needs.
  • Maintain accurate and detailed records of patient interactions, treatments, and progress.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  • Participate in interdisciplinary team meetings to discuss patient care and address concerns.

Competitive Compensation:

We offer an attractive hourly rate of $35-$41 per hour, reflecting your expertise and dedication to patient-centered care.
