Lead Care Manager II

Job Summary: We are seeking a highly skilled and compassionate Lead Care Manager II to join our team at the Central California Asthma Collaborative. In this role, you will work closely with health plans, healthcare providers, and other contracted entities to implement Enhanced Care Management (ECM) services and support programs.
Key Responsibilities:
- Schedule and conduct ECM visits, including patient health assessments, needs assessments, and other assessment tools as needed.
- Track and schedule follow-up appointments with clients, maintaining confidential records containing client information.
- Conduct regular reviews of patient charts to ensure quality services are provided and documented by Lead Care Managers consistently and accurately.
- Work with the Clinical Consultant to recommend and coordinate interventions and referrals, reporting back on their success.
- Coordinate clients' additional providers and services to ensure all of their needs are met.
- Coach clients in learning and developing daily living skills needed to improve and sustain independent living.
- Document assessment and client information using appropriate report forms and/or electronic health records in accordance with office standards and regulations.
- Assist the leadership team in preparing and submitting monthly, quarterly, and/or six-month progress reports to various contractors and others as required.
- Bridge community, cultural, linguistic, socioeconomic, and educational barriers to encourage self-care and participation in the program.
- Attend on-going CCAC in-service trainings.
- Perform all other CCAC duties as assigned.
Requirements:
- Associate's degree (bachelor's preferred) in healthcare, behavioral health, including social work, or related field(s).
- 4 years' experience in case coordination or management working in a healthcare setting. Behavioral Health experience preferred.
- Experience supervising personnel staff preferred.
- Demonstrate competency in communication skills.
- Experience working with community-based organizations preferred.
- English and Spanish fluency required.
- Demonstrated commitment to community education, environmental and social justice, and healthcare access.
- Demonstrated attention to detail, organization, reliability, flexibility, and professionalism.
- Must demonstrate the ability to be culturally competent in all respects.
Travel Requirements:
- Approximately 40% of the time, travel to assessments and various meetings as needed.
